User journey planner?

I like the way this lady thinks and i think we could take it a few steps further, but i will explain later. See if you can understand what she is talking about:

In a campaign, we always give supporters next steps to take action. However, it’s sometimes hard to assure that after completing a certain stept, all the other actions are offered to the supporters.

Is there a tool that allows the mapping of activation like this? Much like a project management tool.
For instance: in this tool, I would list all the different actions we offer. Then, I would start entering the different roads for a supporter to take one of these actions, say subscribing to a newsletter.
I would thenĀ enter all the steps it takes to subscribe to a newsletter. Then this tool would automatically promt a list with links (I imagine a template with “Follow us on Twitter-icons” etc.) to use on the thank-you page, once newsletter-registration is finished.
This would make it so easy to assert that all actions are offered where you want. This could then be mapped in a data visualisation or tree-structure for example.
Does a tool like this exist (or does someone see money and this and wants to develop one? ;-)
